Dukes B3 and c Colorectal Cancer

About 45% of patients who get colorectal cancer will die of metastatic or locally recurrent disease.

Randomized trials have compared perioperative portal infusion of 5-FU starting during surgery and continued for 7 days, and shown no advantage in terms of liver metastases. The NSABP C-02 study showed a survival advantage (p=0.01), as did a study from England (p=0.03; Lancet 340:502, 1992).

It has been hypothesized that since the benefit seen in portal infusion patients occurs after 3 years, while that for standard adjuvant therapy is seen earlier, that there are perhaps 2 different mechanisms of action in these approaches. The CALGB trial 9395 is based upon combining perioperative with postoperative adjuvant treatment to see whether this is the case.
