Head Injury

This case is a 25 year old male who was involved in motor vehicle accident that resulted in a severe head injury. He was intubated to protect his airway. (MISCIMAGE)

One week after admission to the surgical ICU, he developed severe hypoxemia and required mechanical ventilation. His chest x-ray was essentially unchanged and a pulmonary angiogram was requested.

The left pulmonary artery was injected first which demonstrated completely normal pulmonary vasculature.

The right pulmonary injection showed a complete occlusion of the right upper lobe pulmonary artery.

25 year old male who was intubated and mechanically ventilated after a severe motor vehicle accident. He presents with severe hypoxemia.

Case Assessment
This patient presents with a common predisposition to deep venous thrombosis and pulmonary embolus, bed rest. The common predisposing factors to deep venous thrombosis are, immobilization, surgery, fracture, malignancy, thrombophlebitis, trauma, estrogen therapy, obesity, myocardial infarction, and stroke.
